The PreventT2 Diabetes Program
PreventT2 Diabetes is a 26-week Educational Program
Prevent T2 is the National Diabetes Prevention lifestyle change Program, an evidence-based curriculum focused on helping participants make positive lifestyle changes such as eating healthier, reducing stress, and incorporating more physical activity and movement into their daily lifestyle.
Prevent T2 is a year-long program delivered to Douglas County residents in person. The program includes at least 16 weekly sessions during the first 6 months and at least 6 monthly sessions during the second 6 months. Total program participant commitment is approximately 26 hours annually.
A CDC trained Lifestyle Coach, a member of our Outreach Team, will help participants learn new skills, encourage them to set and meet goals, and keep them motivated. The coach will also lead discussions and help make the program fun and engaging.
There will also be a support group of people with similar goals and challenges. Together, the group can share ideas, celebrate successes, and work to overcome obstacles. It may be easier to make changes when you’re working as a group than doing it on your own.

Where Can I Participate in a PreventT2 Program?
Participate in the Prevent T2 program by registering online, giving us a call, or stop by our Outreach office. Registration is FREE!
The PreventT2 program is scheduled to take place over 26-weeks. That’s 26 1-hour sessions where participants learn about preventing type 2 diabetes through healthy lifestyle changes.
We will meet once a week for the first 16-weeks then bi-weekly for the remainder of the course.
Classes are scheduled to begin on April 2, 2024 and will progress through November 19, 2024.
What to Expect in our Prevent T2 class
Our diabetes prevention program, Prevent T2, is a CDC evidenced-based educational program. The Prevent T2 program is a lifestyle change program, delivered over a 26-week period, designed to help individuals with prediabetes or those at risk of developing type 2 diabetes make healthy choices to reduce their risk of developing the condition. This program aims to educate participants about evidence-based strategies for improving their overall health and preventing the progression to type 2 diabetes. Here are some key components and benefits of the Prevent T2 program:
Education: The program provides participants with education on healthy lifestyle changes. This includes information on nutrition, physical activity, stress management, and other behaviors that can help reduce the risk of type 2 diabetes.
Lifestyle Changes: The program encourages participants to incorporate the knowledge they gain into their daily lives. This involves making sustainable changes to their diet and physical activity routines to promote better health.
Family Impact: Healthy lifestyle changes promoted by the Prevent T2 program can have a positive impact on families. When individuals make healthier choices, it often influences the choices and behaviors of their family members, fostering a healthier environment for all.

What Is the Benefit of Being Part of a PreventT2 Program?
As part of a PreventT2 group, you will work with other participants and a trained lifestyle coach to learn the skills you need to make lasting changes. These changes include losing a modest amount of weight, being more physically active, and managing stress.
Being part of a group provides support from other people who are facing similar challenges and trying to make the same changes you are. Together you
can celebrate successes and find ways to overcome obstacles.
What Is the Format of the PreventT2 Program?
PreventT2 groups meet for 26 weeks over the course of one year — weekly for the first 16 weeks, then once or twice a month for the for the remainder of the program to maintain healthy lifestyle changes. During each session, your lifestyle coach will teach a lesson and lead a group discussion.
For example, you will learn to:

• Add physical activity to your life
• Manage stress
• Stay on track when eating out
• Understand and respond to your food cues
• Stay motivated
• Solve problems that can get in the way of healthy changes
